Fungal Nails

Diagnosis and Treatment

Fungal nail infection is a slowly developing invasion by fungal known as dermatophytes, which thrive in moist warm skin conditions and digests skin. The fungus attacks the nail bed and causes the nail to become thick and to separate from the nail bed. Often found as a secondary infection from Athletes foot (tinea pedis), this is also a dermatophyte and it is usually found interdigital in the web spaces of the toes. Infection can easily spread from skin to nail, nail to nail and nail to the skin.

Early signs to be aware of are:

✔ Discoloration in the side of the nail, which slowly creeps down towards the nail base. The discoloration may be creamy-white, yellow or brown.
✔ Infections may be odorous
✔ The nail may become brittle and break or crumble easily.
✔ Thickening of the nail.

While athletes Foot is likely to respond to topical anti-fungal creams, however, fungal nails never go away of their own accord. The longer the fungus is left, the more damage is done and the more difficult they become to treat. If the fungus is caught in the early stages, weekly application of terbinafineanti-fungal nail lacquer, along with across regular reduction of the thickened nail plate, will probably be the best course of treatment. If the base of the nail is infected then systematic medication (prescribed by your GP) is required, unfortunately, there is no quick cure, fungal nails can take anything from six to eighteen months to resolve and outgrow.

Fungal nails can be confused with other conditions such as psoriasis or a damaged nail, The Laurels Clinic have a professional connection with the Microbiology Department at St. James Teaching Hospital, Leeds, where they can offer a quick diagnosis by taking samples of nail clippings and skin for microscopy examination and culture to determine the presence of fungal spores. Results are then sent to your GP where a full assessment will be given to ascertain if you are suitable for any oral or topical medications that may warrant prescribing.

Elaine is able to offer the Lacuna method to deliver a more stable topical solution to the nails by micro drilling to treat the infection. This relatively new treatment is offering positive antifungal The treatment involves drilling minute hols treat the nail and spraying topical terbinafine across the nail plate then we request you do the same Department and comply with treatment for the best possible result. Terbinafine is an antifungal drug and creates a flora imbalance in the cell wall causing the fungi to die. Treatment costs vary dependant upon the number of infected nails to be treated but start costs are £…. treatment is usually painless and requires no detriment or downtime.

In addition, Elaine is able to offer MLS Laser Therapy which is a completely safe non-invasive and painless method of treatment. The laser supplies light energy directly to your cells which encourages your body’s own healing process. An initial consultation is required and a full medical history to determine suitability.

Gait Analysis

Biomechanical Assessment and Gait Analysis

✔ Plantar Fasciitis (heel pain)
✔ Achilles Tendonitis
✔ Painful Bunions
✔ Mortons Neuromas
✔ Metatarsalgia (forefoot pain)
✔ Aching feet (muscular and or joint pain)

All of the above conditions are related to the mechanical forces in the lower limb. The podiatrist will assess how you walk and offer a clinical examination of the underlying structures of the foot function to identify factors that may be causing or contributing to your pain or discomfort. That information is then used to formulate a treatment plan unique to you and may involve recommendations on footwear and hosiery, sports shoes, specific exercises, and insoles also known as orthoses to improve foot function, amend mobility and alleviate pain by deflection of pressure from any problem areas.

The Laurels Clinic has a professional relationship with both the NHS and has a dedicated team that custom makes orthotics by prescription for you.

Nail Surgery

Nail Surgery under Local Anaesthesia

In growing nails is a painful condition by which the nail edge penetrates the surrounding skin. A splinter of nail pierces the skin and the area becomes tender and swollen frequently accompanied by secondary infection and hyper granulation tissue. You may be prescribed antibiotics for the infection but this does NOT cure the problem, and only relieves it temporarily. The condition will soon reoccur unless the splinter is removed. The podiatrist can offer you a procedure known as nail avulsion to remove the spicule and narrow the nail plate by using a chemical to eradicate the nail cells causing the problem. This procedure is done partially and leaves some nail intact for aesthetic reasons or maybe a total nail avulsion where the nail is removed and chemicals applied to ensure that no nail growth occurs. This simple yet effective procedure is carried out in the clinic under local anesthesia for minimal discomfort.

Every effort is made to make sure that a cosmetically aesthetic result is achieved.

Podo Paediatrics

At the Laurels Children’s Podiatry Clinics we have a special interest in the growth and development of children’s feet. Pain in a child’s foot, ankle, knee, hip or lower back is never considered normal. We don’t believe in “growing pains”, and pain that lasts more than a few days, or that is severe enough to limit the child’s walking, should be evaluated by an appropriately qualified Podiatrist.

Designed For Special Care

A Newborn’s feet are soft and pliable because they’re made mainly of fatty tissue (muscles, ligaments, and tendons) and soft cartilage, which changes into the bone as their feet grow. A child’s foot will double in size by the age of one, and they are approximately half their adult length by 18 months of age. Your child’s feet are literally the foundation of their body. They are a complex network of bones, joints, tissues, muscles and nerve endings that all work together to coordinate motion in the rest of their body. When something is out of balance in that complex network, the impact can be felt throughout their whole system. It is important to always treat foot pain to avoid life long problems that may occur as a result.

Podo Paediatric Conditions We Treat

Elaine and her team have excellent referral pathways both NHS and private for any secondary opinion or differential diagnosis.

Verrucae Needling

Verrucae

Verrucae are benign tumors caused by the human papillomavirus HPV and are very contagious. They are often contracted from moist or wet environments like swimming pools and gymnasiums. The virus invades broken skin and incubates itself and may lay dormant for weeks or months. It replicates and causes overproduction of cells resulting in a clumpy warty appearance. Though benign it is always best to have them professionally checked due to increased incidences of skin cancers. The virus can only survive in the epidermis. If the virus is asymptomatic (does not hurt) and is not spreading, we tend to leave them alone where the spontaneous resolution will occur. They can linger for a number of weeks, months or even years. Your podiatrist can help you eradicate them should they become a nuisance or painful.

Needling

The Laurels clinic and Elaine Broadbent are one of the few podiatry clinics in the country to offer this technique. It is believed to work by destroying the verrucae cells and it creates a controlled inflammatory response to stimulate the body’s autoimmune system to create antibodies that destroy the virus. The treatment has been successfully used for a number of years worldwide. It requires only one appointment and dressing for 48 hours postoperatively. A shower can be quickly taken after 24 hours if you wish.

The procedure is carried out under local anesthetic. The verrucae are pierced multiple times with a sterile needle so that the microscopic viral particles are exposed to the sub-dermal cutaneous tissue underneath the skin (the epidermis). This triggers an immune localized cell response. Even when there are multiple verrucae lesions the podiatrist will usually only treat one area. The other satellite verrucae usually spontaneously regress following AIST (autoimmune stimulation therapy). The success rate in clinical trials and the audit is approx 75-80% resolution.

Contrary to Walt Disney and folklore, Kissing frogs does not get rid of warts, nor does it guarantee you a prince either.

Microsclerotherapy

Microsclerotherapy and its use in treatment for spider veins and broken capillaries

If you suffer from thread or spider veins, microsclerotherapy is a safe and proven option to reduce their appearance and help you to restore lost confidence. Thread or spider veins, also known by its medical term “telangiectasia varicose veins”, are red and irregular in appearance. They are the result of very fine dilated veins lying just beneath the surface of the skin. The condition most often affects the lower limbs especially in women due mostly to hormonal changes such as pregnancy and menopause and also with hereditary conditions and vascular incompetence affects legs, but can occasionally be found in other parts of the body.

If spider veins are cause of concern for you, then there is no need to worry. Your situation will be assessed by our expert podiatrist health care professionals who specialise in the lower limb during your comprehensive consultation. We are registered with HEALTH CARES PROFESSIONALS COUNCIL, HCPC and The Royal Society of Podiatrists for your safety and wellbeing.

  • How does it work?

With microsclerotherapy, carefully injects a substance, Sclerovein, directly into the veins, causing them to shrink. This product is safely absorbed by your body over a period of a few weeks. Microsclerotherapy is a very simple and straightforward procedure. It is extremely effective, too.

  • Clinical effectiveness in microsclerotherapy

You should see some immediate improvement though this will improve over the next few weeks . Overall up to 75%  of patients will see an improvement in one session though further treatments may be required.

  • Recovery and healing times

Following each session, you will be given compression stockings to wear continuously for several days to support the treatment. You may experience some brown discolouration of the skin which can appear for up to 3 months, this will fade though occasionally and rarely may be permanent . You will be able to drive immediately after the session but baths or showers and alcohol are best avoided for 24hrs . Normal exercise can be resumed after 24 hours, including swimming.

  • Is the procedure painful?

The procedure is slightly uncomfortable, but your clinician will cause minimal discomfort and anaesthetic is not required. You may be advised to take analgesic medication post operative for your own comfort in discharge.

  • How many treatments are required?

Microsclerotherapy treatment may need to be repeated, usually after six weeks. Most or all vessels treated during the course of microsclerotherapy will be removed. However, response differs between patients and you may need between one to four treatments.

This treatment may be suggested for the following Thread Veins.

Reconnect with a more confident you!

Up to 70% of the population are affected by these changes to vascular function and confidence can be greatly affected. By eliminating the spider veins a more youthful and pleasing aesthetical appearance is achieved. Giving you most confidence and allowing you to wear those shorts or flowing summer dresses with confidence.

An initial consultation is required and a full medical history taken to ensure suitability to the procedure . These veins are usually not harmful or painful but can affect your confidence and may stop you wearing what you want. Thread veins are just small blood vessels which sit at the surface of the skin, most commonly found on the legs or face.

Laser thread vein removal is carried out in complete comfort and safety using industry-leading technologies (Candela GentleMax Pro) designed with sensitive skin in mind.
